Immunization Requirements

All students born on or after January 1, 1957 who are registered for six or more credits are required to comply with NYS immunization requirements.

Required Immunizations

MMR

The MMR (Measles, Mumps and Rubella) is often given as 2 immunizations in childhood. It includes:
2 Measles immunizations
1 Mumps immunization
1 Rubella immunization

OR

A blood test (known as a blood titer) for measles, mumps and rubella showing immunity, lab report with your name and results must be attached.

Meningitis

For this requirement, you need to show that you've received:

ACWY: Menactra, Menveo, MenQuadfi vaccine within the last five (5) years
OR
Meningitis B: 2 to 3 doses of Bexsero or Trumenba at any time
OR
Decline the meningitis requirement
. There is no penalty for declining these immunizations. Completing the waiver form with the student's signature and date ensures compliance with NYS Public Health Law. Students may receive the vaccine at any time in the future through their healthcare provider or at a local pharmacy. Please make sure you have read about the meningitis illness (see links below). Immunization FAQs

You can contact your primary care provider, high school, previous job in healthcare, current/previous college(s), or military to obtain your immunization records.

We will accept any one of the following documentation for your MMR vaccinations:

A copy of your immunizations (including on an official government/school letterhead – the simplest place to obtain this may be from your most recently attended high school or college; OR

A copy of your immunizations (including actual dates) on provider’s letterhead, which includes printed name, address, and telephone number; OR

A blood test to confirm immunity. Documentation of your name and lab results must be included to be accepted.

In accordance with CDC recommendations and the evolving public health situation, the College is strongly encouraging everyone in the community—students, faculty, and staff—to stay up to date with COVID-19 vaccinations, and will continue to organize on-campus vaccination clinics whenever possible based on vaccine availability. However, the College will no longer require students to submit COVID-19 vaccination data. If you are in a health related program please refer to their requirements for additional information.

If you would like your immunization records sent to a new school, medical provider or employer please complete and submit an immunization records release form. This form may be submitted to the Health Office via any of the following methods:

Once we receive your form, it will take 24-72 hours for us to process your request.
