GO TO FORMSThe Office of Accommodative Services (OAS) is committed to providing equal access and an inclusive campus community by providing accessibility services and advocacy for individuals with disabilities.
Getting Started
- Submit a DCC admission application, following directions of Admissions Office for enrolling at DCC.
- Complete the Student Accommodation Request Form. You will need your student identification number (A number). If you do not have this yet, please enter your first name, middle initial and last name.
- Submit documentation of your disability/condition that substantially limits one or more major life activities. This might be an IEP/504 plan, psychoeducational evaluation, speech/language evaluation, documentation from a therapist, neurologist or other medical professional. Please review our documentation requirements. Please note: Do not wait to register with our office due to not having proper documentation. Complete the request form, and we will respond with information about next steps, which may include completing the Self-Assessment Survey.

These meetings are held for incoming students once they have registered for classes.
